Adult Case Management     

 

Case Management uses a modified approach to the psychosocial rehabilitative model of care for consumers with severe and persistent mental illness. Treatment is community based and focuses on assisting consumers increase their ability to manage their illness and improve the quality of their lives while residing in their own community. Case managers help consumers access needed benefits, acquire or maintain housing, and develop the necessary skills to manage symptoms of their illness. Also addressed are problem solving skills, socialization, and educational and vocational needs. Case managers and consumers work together to develop individualized goals and objectives for treatment, including coordination and collaboration with the consumer’s family members, community partners and other natural supports.

Specific case managers are assigned to each of the Congregate Care Facilities (CCF) in Spokane County. This allows case managers to develop a better relationship with the CCF managers, enhance their knowledge of the CCF home and its needs, and it streamlines communication. This also allows for better continuity of care by having one case manager available for crisis management.

The evidence based treatment of Multifamily Group therapy is available for consumers who suffer from schizophrenia or schizoaffective disorders. It provides education to the consumer and his/her family on crisis management, coping skills, symptom management, and other relevant issues that may impact their lives due to the consumer’s illness. Additionally, Adult Case Management offers psychoeducational and support groups for Men and Women living with illness, co-occurring disorders, and Life with Schizophrenia.

Bachelor's and Mater's level staff who are supervised by mental health professionals provide these services.
